Friends and Family Test
The Friends and Family Test is a quick way to tell us about your experience of our services.
What is the Friends and Family Test?
The Friends and Family Test gives patients the opportunity to give anonymous feedback after receiving NHS care or treatment. It helps us understand what is working well and where we need to improve.
Giving feedback is optional.
How to give feedback
You can complete a Friends and Family Test paper form when you visit one of our practices.
Forms are available in our waiting areas. You can also ask reception for a form.
If you need help to complete the form, please ask a member of staff. A friend, family member or carer can also help you.
What we ask
The Friends and Family Test asks: ‘Overall, how was your experience of our service?’
You can choose an answer from very good to very poor. You can also tell us in your own words what was good or what could be improved.
What happens to your feedback?
We review feedback to help us understand patient experience and improve our services.
We submit Friends and Family Test data through the national NHS reporting process.
