The Care Home Project is our long-running programme that brings our GPs, nurses and wider teams closer together with the local care homes we support, so residents get proactive, joined-up care where they live. It has been running for some time and continues to grow.
What this means for residents
Working alongside care home staff, our teams keep a closer eye on residents' health day to day. That means:
- Regular reviews of residents' health, medications and care plans
- Care plans agreed together with residents, families and care home staff
- Clearer routes to the right clinician when something changes
Fewer avoidable hospital trips
Spotting problems early often means a resident can be treated where they live, rather than making an unnecessary trip to hospital. The project does exactly that — with better sharing of information (within the usual confidentiality rules) and quicker follow-up when something looks wrong.
