When should I register with a new GP?

You may need to register with a new GP practice if you have moved to Leicester, moved to a different part of Leicester, or your previous GP practice no longer covers your address. Registering with a local GP can help you access NHS GP services near where you live.

What you may need to register

The NHS GP registration form may ask for details such as your name, date of birth, address and previous GP practice. If you know your NHS number, this can also help, but you can usually still complete the form if you do not know it.

  • Your current address.
  • Date of birth.
  • NHS number, if known.
  • Previous GP details, if known.
  • Details of regular medication or ongoing health conditions, where relevant.

Repeat prescriptions when moving GP

If you take regular medication, do not wait until you have run out before starting registration. If you are between GP practices, contact your current GP practice or a pharmacist for advice about your medication while your registration is being reviewed.

If you have moved with children or other family members, each person may need their own registration. Choose the Willows Health practice that covers your address and complete the NHS registration form for each person who needs to register.
