Birthday MOT for patients with long-term conditions

If you have a chronic or long-term condition such as diabetes, asthma, heart disease or high blood pressure, your annual health review will now be scheduled based on your birthday month.

We call this your "Birthday MOT" — a dedicated time each year to check in on your health, review your medications and make sure everything is well-managed.

This makes it easier for you to plan ahead and helps us provide timely, personalised care. You'll receive a recall or invitation near your birthday with details on what to expect and how to book.
